For use when any injury occurs to a player that requires the player to be transported to a hospital or when an ambulence or paramedic is called. This form must be turned into the President of Utah High School Hockey. |
| Player Transfer Request & Authorization |
All Transferring Players |
A player may not transfer from one UHSH ice hockey program to another without authorization. All transferring players must complete this form and submit it to the UHSH Registrar immediately upon transfer. |
